Cook and Dunagan provide excellent allergy and asthma care. Every effort will be made by the entire office staff and physicians to give you the best medical care while at the same time treating you with respect and kindness.

About Cook and Dunagan

Hours of Operation:

Office Hours: The office is open Monday through Thursday from 8:30am to 5:30pm, and Friday 8:30am to 12pm. A Physician is on call at all times for emergencies.

Injection Hours: Allergy injections are given Monday through Friday from 9:00 a.m. -12 noon and Monday through Thursday from 2:00 p.m.to 5:00 p.m. No appointment is necessary for injections. Extracts may be ordered during office hours. Please allow the nurse 14 working days to prepare the extract.

Appointments: Appointments may be obtained by calling (706) 733-1104 during office hours. Every effort will be made to provide the earliest possible appointments for the convenience of the patient. When calling for an appointment please furnish the receptionist with the reason or the need for the visit or the nature of your problem. This is very important in order to allow us to give priority to your needs as well as to reserve sufficient time for your visit with us. Your new patient appointment time will be an approximate time and will last a minimum of two hours. Due to the unscheduled nature of emergencies, occasional delays do occur. We hope that you will understand that these delays are unavoidable. If you are unable to keep a scheduled appointment, please call and cancel as far in advance as possible, so that we may be able to give this time to another patient.

Telephone Calls: The office staff members are well trained to handle phone calls and will answer your questions or will obtain your chart and relay an answer to you from your doctor. Please make your questions and requests as specific as possible. Calls are taken by the staff and relayed to Dr. Dunagan twice daily, except for emergencies, where they are handled immediately.

Please have your pharmacy call for prescriptions and prescription refills. You may also pick up prescriptions in person at the office. When calls are made regarding medications, the name, strength and dosage schedule should be provided. Calls should be made during office hours so that your chart may be reviewed.

Hospitalization: If you require hospitalization or if Dr. Dunagan is asked to see you for consultation while you are hospitalized, you will receive a bill from our office separate from the hospital bill. Please contact this office with your insurance information, so that we may file these charges for you. If pre-certification is required by your insurance company, notify this office prior to admission.

Office Activities:

Initial allergy history and physical exams, periodic follow-up examinations, allergy and immunology consultation, allergy skin testing, allergy injections, and pulmonary function testing are performed in the office. Most labs and X-rays are performed at an off site facility. If results of recent x-rays and laboratory data can be obtained prior to your evaluation, this information would be extremely helpful if brought with you at the time of your first visit.

Payment Information:

Insurance: Dr. Dunagan participates in most managed care plans as well as Georgia Medicaid, (On-Physician) referral, Medicare, Blue Choice, Aetna, University Healthlink, Lifeplans and Healthnet. Benefits and co-payments may vary. Insurance is filed at no charge for initial visits. Check with your managed care plan administrator. Many plans require a written referral from the patient's primary care physician.

Fees and Billings: The responsibility for payment of our fees is the direct obligation of the patient. We appreciate payment when services are rendered but also realize that illness does not always occur at a convenient time. If you have questions, please do not hesitate to ask.

Fees are based on reasonable and customary charges for this area. The fee for the first visit or a new problem is higher than a routine follow-up visit, because more time is required.

An estimated fee for any procedure will be given on request by our business office. If you are unable to pay in full at the time of service, you should pay your balance within 30 days.

If you have a financial problem, we will understand. Contact our office manager and time will be extended for payment which is fair to you and the clinic.
